To save time during your visit, download and fill out your patient forms now. The first thing you’ll do when you arrive for your initial visit is fill out your registration forms so the office team can get to know you a little better. It’s always a good idea to arrive a few minutes early.

Your first visit will include:

  • Registration -- At this time we will discuss your insurance information and develop your patient chart. Download and fill out the on-line registration forms in advance to shorten your registration process.
  • X-rays -- It is impossible to see directly between the teeth or under the gums or bone without the use of a dental radiograph. Many times the patient has no signs or symptoms of disease. Dental x-rays can detect disease much sooner than the clinical examination alone. This enables the dentist to identify problems early so that pain and extensive, costly treatment can be avoided.
  • Examination and Review of X-rays -- Most ForwardDental offices either have, or will be transitioning to digital radiography which allows our patients to view their X-rays immediately.
  • Recommended Treatment Plan -- Your dentist will discuss your recommended treatment plan and involve you in the treatment process.
